Reiki, Energy Healing & Activity Waiver

By participating in any session, workshop, class or experience with Susan of Susan Ray LLC, you acknowledge that:

• You are 18 years of age or older

• You have read and understood the waiver and release of liability outlined below.

WAIVER AND RELEASE OF LIABILITY

I understand that meditation, breath work, Kundalini Yoga, and Reiki are simple, gentle, energetic techniques used for stress reduction and relaxation. I understand that Susan Ray does not diagnose conditions nor does she prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional. I understand that Susan Ray, or any activities led by Susan Ray, do not take the place of medical care. It is recommended that I see a licensed physician or licensed health care professional for any physical or psychological ailment I may have. I understand that meditation, breath work, Kundalini Yoga, and Reiki can complement any medical or psychological care I may be receiving. I also understand that the body has the ability to heal itself and to do so, complete relaxation is often beneficial. I acknowledge that long term imbalances in the body sometimes require multiple sessions in order to facilitate the level of relaxation needed by the body to heal itself. I understand that the practitioner will be remotely sending energy to me for the duration of my session, workshop, class, or activity.

I assume sole responsibility for my own health and for the results of any sessions provided by Susan Ray that may affect my health in any way. Treatment(s) will not replace conventional medical diagnosis or treatment. I will continue taking medication prescribed by a licensed medical physician and will continue to follow his/her instructions. I release Susan Ray from all legal liability during my participation in any session, class, workshop, or activity led by Susan Ray. All information received by me from Susan Ray is accepted with full knowledge that any action taken by me as a result of the information received is my complete responsibility.

Having read, completed and understood the foregoing, I request to participate in the session, workshop, class, or activity led by Susan Ray. I understand that Susan Ray is providing this service for me at my request, and is not responsible for the outcome of the session. I agree to hold Susan Ray harmless for any intended or unintended result.
